Our Medicines Management Team plays a vital role in ensuring that medicines are used safely, effectively, and appropriately for our patients. Working closely with GPs, nurses, and the wider practice team, they help optimise treatments, reduce risks, and improve long-term health outcomes.

How we support patients

  • Medication reviews – checking that medicines are still the best choice, at the right dose, and working well for you
  • Long-term condition support – helping manage conditions such as high blood pressure, diabetes, asthma, and heart disease
  • Reducing side effects and interactions – identifying and addressing potential problems between medicines
  • Stopping unnecessary medicines – safely reducing or deprescribing where appropriate
  • Advice and reassurance – answering questions about how and when to take medicines, and what to expect

How we support the practice

  • Prescription safety – ensuring prescribing follows national guidance and best practice
  • Medicines optimisation – making sure treatments deliver the best outcomes with the least burden
  • Clinical audits and monitoring – supporting quality improvement and patient safety
  • Liaison with community pharmacies and hospitals – helping ensure smooth transitions of care
